Client Service Agreement

This agreement sets forth the expectations and standards that both client and coach expect during the coaching relationship. Policies and expectations are outlined in the following:

1. The Relationship Between Client and Coach

As the Client’s Life Coach, ___________________________________, doing business as empowerMEnt, (hereby referred to as “coach”) will partner with the client on a journey of discovery, self-exploration, accountability, and follow through. The coaches’ job is to help discover, create a plan, and the ability to obtain the goals the client sets forth. The coach will help the client discover within themselves the beliefs or issues that are blocking the client from moving forward in achieving their goals.

*The coach operates from a particular coaching model, but it is the responsibility of the client to follow through with implementing the plan set forth between the coach and client.

2. Terms of Service

Achieving the results desired and expected by the client and coach takes time.

 

In order for the coaching to be most effective and to allow time for changes and results to occur in the clients life and best interest, it is recommended that the client commit to a minimum of 3 months of coaching.

 

Either party may terminate the coaching relationship any time with the agreement to have an exiting session before concluding the relationship.

 

There is no refund for any unused sessions and sessions are not transferable.

3. Scheduled Sessions​

Regular coaching sessions will be scheduled for days and times and be via Zoom or phone. Session times and costs are included on the booking page. Packages are offered at a lower rate.

All sessions must be prepaid

4. Cancellation Policy

The client is responsible for any regular session fees if the client does not cancel a minimum of 24 hours prior to the scheduled session time. Cancellations must be by phone not text or email.

5. Additional Calls

Unscheduled calls or emergency sessions will be billed at regular rates.

6. Confidentiality

The clients identity, relationship and context of the sessions are strictly confidential except in a situation where such confidentiality would violate the law or cause harm to the client or another person or place.

7. Payment Methods

empowerMEnt Life Coaching accepts payment via Venmo or Messenger Payments unless otherwise discussed and agreed upon.

8. Liability

Client agrees and understands that empowerMEnt and it’s life coaches and business consultants are not medical doctors, licensed therapists or psychologists. We are not a substitute for psychotherapy or medical treatment. If a situation arises where medical or psychological intervention is needed, empowerMEnt is ethically and legally bound to refer the client to a mental or medical professional.

I have read and understand this agreement and abide by its terms and conditions, and acknowledge receipt of my own copy of this agreement.

Consultant Client Service Agreement

This agreement sets forth the expectations and standards that both client and coach expect during the coaching relationship. Policies and expectations are outlined in the following:

1. The Relationship Between Client and Consultant

As the Client’s Business Consultant ___________________________________, doing business as empowerMEnt, (hereby referred to as “consultant”). empowerMEnt will partner your company ________________________________________ to create a cohesive and comprehensive plan to get your business up and running, back on track, rebranded and/or organized. This includes your staff, vendors, inventory, marketing, logistics and sales.  The consultant will help discover and uncover any areas that need to be addressed and propose a plan to implement for your success. The consultant operates from a particular consulting model, but it is the responsibility of the client to follow through with implementing the plan set forth between the consultant and the client.

2. Terms of Service

Achieving the results desired and expected by the client takes time.

 

To be most effective and to allow time for changes and results to occur in the clients business and the companies best interest, it is recommended that the client commit to a minimum of ________ (months or weeks) of consulting and implementation.

 

Either party may terminate the business relationship any time with the agreement to have an exit session before concluding the relationship

empowerMEnt Life Coaching accepts payment via Venmo or Business Check, unless otherwise discussed and agreed upon. Any insufficient funds will be charged an additional $50 fee per deposit with only 2 days in between deposit attempts


8. Liability

Client agrees and understands that empowerMEnt and its business consultants are not liable for the success or failure of their business.
